- Fill out and submit a Hardship Permit Application, DMV Form 735-6044
- Pay the $50 non-refundable application fee
- Pay a $75 reinstatement fee
- Obtain a recommendation from the convicting judge*
- Submit an SR-22 Certificate of Liability for insurance
- Install an ignition interlock device Oregon approves for the duration of the Hardship Permit period
*This is not required under Implied Consent, a law which states that if a police officer pulls a driver over for suspected DUI, the driver is deemed to have given implied consent to test their blood alcohol content.

IIDs Oregon requires are leased, not purchased. It’s best to lease from a trusted provider, as purchasing a device outright would cost thousands of dollars.
The cost to lease your IID will vary depending on your needs, but Intoxalock devices typically cost between $2 and $3 a day, with an optional protection plan. State specialists will help you find any possible discounts or financial assistance as well. Intoxalock also offers a free rebate of $100 following installation. Rebate paperwork will need to be mailed in and verified before the credit is applied.
